Provider contract: Please request a contract by email, complete a Provider Contract Inquiry Form (PDF), or call your designated Provider Services Account Executive.
As a reminder, an OH|ID is required for providers to access the Provider Network Management (PNM) module. If you do not have an OH|ID, you will need to create an OH|ID to access the PNM module. If you have an OH|ID, or once you have created one, use your new OH|ID to log in to the PNM module.

| Disease management and healthy behaviors programs. | We support your care for our members through programs that address physical health conditions such as asthma, depression, diabetes, and/or coronary artery disease, those living with trauma exposure, and/or those with unmet social needs. We encourage members to actively participate in improving and maintaining their health, and we reward them for practicing healthy behaviors. |
| Behavioral health resources and support. | We recognize that physical health is just one part of wellness. That is why we integrate behavioral health services and administer them directly.
